What is type 2 diabetes?

GLP-1 receptor agonist

How it works

Mimics the incretin hormone GLP-1 to stimulate insulin release after meals, reduce glucagon, slow gastric emptying, and reduce appetite. Shown to reduce cardiovascular events and slow kidney disease progression.

BC PharmaCare coverage

Listed on BC PharmaCare with Special Authority for eligible patients with T2DM. Criteria include inadequate control on metformin ± other agents. No generic currently available — brand cost applies above any deductible.

  • Once-weekly subcutaneous injection (pre-filled pen)
  • Also promotes weight loss — particularly relevant for patients with obesity
  • Must be stored refrigerated until first use
  • Special Authority from prescriber required for PharmaCare coverage

Empagliflozin

Jardiance

SGLT-2 inhibitor

How it works

Blocks the SGLT-2 transporter in the kidney, causing excess glucose to be excreted in the urine rather than reabsorbed. Also reduces sodium retention, lowers blood pressure, and has proven heart failure and CKD benefits.

BC PharmaCare coverage

Listed on BC PharmaCare with Special Authority for T2DM with cardiovascular disease or high risk, or for heart failure and chronic kidney disease indications. No generic currently available.

  • Once-daily oral tablet
  • Also approved for heart failure and chronic kidney disease (separate indications)
  • Increases risk of genital yeast infections and urinary tract infections
  • Do not use if eGFR is too low — check with your pharmacist

Is metformin still the first-line treatment for type 2 diabetes in Canada?

Yes. Metformin remains the recommended first-line medication for most adults with type 2 diabetes in Canada, per Diabetes Canada guidelines. It is well-tolerated, inexpensive, and broadly covered by BC PharmaCare. GLP-1 agonists and SGLT-2 inhibitors like Ozempic and Jardiance are typically added when additional glycemic control, cardiovascular protection, or weight management is needed.
